To All CRUDEM Volunteers:

We write to inform you of two items related to CRUDEM’s policies and practices. The first concerns the volunteer liability waiver. This form has been updated several times to reflect changes in the legal environment. This is a revised waiver that we expect will remain valid for some time to come. Please review this document carefully and return it fully executed to CRUDEM c/o Deb Pane-Motyl at 868 Stony Hill Road, Wilbraham, MA 01095. Effective immediately, this two-page form replaces the previous waiver. Your name must be printed in your hand on the first page, signed and witnessed on the second page. Once you have executed it, it will remain in force throughout your time as volunteer. You will not be asked to execute another liability waiver unless there is a significant change to the law. Please return it fully and properly executed as soon as possible. No volunteer will be cleared to travel to Milot if this revised waiver has not been properly executed and filed with Deb Pane-Motyl at least eight (8) weeks prior to their planned trip to Hôpital-Sacré Coeur.

The second item concerns shipments to Hôpital-Sacré Coeur via CRUDEM’s quarterly containers. In the past, we have shipped materiel to Haiti using Caribbean American Shipping Express in Hollywood, FL. To better manage the process and save money, we began shipping out of the Port of New York & New Jersey last year using Holy Name Medical Center in Teaneck, NJ as the point of origination. Since an increasing number of donors and medical teams have begun shipping supplies to Haiti via the CRUDEM containers, we see a need to make the process more formalized and structured. This should help reduce costs and increase efficiency. The policy set forth here outlines a process that will enable us to better utilize our limited container space and process the necessary customs documentation in a timely manner.

We appreciate your patience and understanding as we seek to manage risk, improve efficiency and better serve the people of northern Haiti.
